**HOOPSTARS BASKETBALL GROUP LLC YOUTH BASKETBALL PARTICIPATION LIABILITY WAIVER**

This Participation Agreement and Liability Waiver (the "Agreement") is entered into by and between HOOPSTARS BASKETBALL GROUP LLC, hereinafter referred to as the "League," and the undersigned participant, hereinafter referred to as the "Participant."

**1. ASSUMPTION OF RISK AND WAIVER**

By signing this Agreement, the Participant acknowledges and understands that playing basketball involves inherent risks, including the risk of injury. The Participant expressly assumes all such risks and waives any claim against HOOPSTARS BASKETBALL GROUP LLC, its organizers, volunteers, and affiliated entities arising from participation in basketball activities organized by the League.

**2. RELEASE AND INDEMNIFICATION**

The Participant releases, discharges, and holds harmless HOOPSTARS BASKETBALL GROUP LLC from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant during participation in the basketball league.

The Participant agrees to indemnify and hold HOOPSTARS BASKETBALL GROUP LLC harmless against any and all claims, suits, or actions arising out of or in connection with the Participant's involvement in the league.

**3. MEDICAL CONSENT**

The Participant acknowledges that they are responsible for their own medical insurance coverage. In case of a medical emergency, the Participant authorizes HOOPSTARS BASKETBALL GROUP LLC to secure medical treatment on their behalf.

**4. CODE OF CONDUCT**

The Participant agrees to adhere to a code of conduct that promotes sportsmanship, fair play, and respect for all participants. Failure to abide by the League's code of conduct may result in disciplinary actions.

**5. IMAGE RELEASE**

The Participant grants permission to HOOPSTARS BASKETBALL GROUP LLC to use, reproduce, and distribute their name, image, likeness, and voice in connection with any promotional materials, broadcasts, or other League-related media.

**6. ELECTRONIC SIGNATURE**

By checking the box above, I acknowledge that I have read, understood, and voluntarily agreed to the terms and conditions of this Agreement.

**7. GOVERNING LAW**

This Agreement shall be governed by and construed in accordance with the laws of the state of Pennsylvania. Any disputes arising under or in connection with this Agreement shall be resolved through arbitration in accordance with the rules of the American Arbitration Association.
